Accident Summary:
ON 9 JANUARY 1970, AIRCRAFT 64-14004, A UH-1B GUNSHIP ASSIGNED TO THE 335TH AVIATION COMPANY (ASSAULT HELICOPTER) WAS FLYING COMBAT ASSAULTS NORTHEAST OF VINH LONG, RVN. THE AIRCRAFT HAD A NORMAL FLIGHT INTO THE VINH LONG POL AREA. AS THE AIRCRAFT WAS DEPARTING POL, CPT DEMERS NOTICED A FLUCTUATION IN THE N-2 ENGINE RPM. SIMULTANEOUSLY, THE CREWCHIEF HEARD A UNUSUAL WHINNING NOISE IN THE ENGINE AND REQUESTED THE AIRCRAFT BE LANDED AT VINH LONG FOR INSPECTION BY AN AIRCRAFT TECHNICAL INSPECTOR. THE AIRCRAFT COMMANDER LANDED THE AIRCRAFT AND REQUESTED MAINTENANCE PERSONNEL TO LOOK AT THE AIRCRAFT. A TECHNICAL INSPECTOR FROM THE 114TH AVIATION COMPANY (ASSAULT HELICOPTER) INSPECTED THE AIRCRAFT. THE TI WAS INFORMED THAT THE ENGINE RPM WOULD NOT EXCEED 6400. AT THIS TIME THE TI ADJUSTED THE LINEAR ACTUATOR TO OBTAIN 6600 RPM. THE TI ALSO NOTICED OIL LEAKING FROM THE N-2 TACHOMETER. THE TI EXPLAINED THAT THE FUEL CONTROL GOVERNOR COULD POSSIBLY BE GOING BAD BUT SAID THE AIRCRAFT COULD BE CLEARED FOR A ONE TIME FLIGHT TO BEARCAT, RVN, THE HOME STATION OF THE AIRCRAFT. THE TI PUT THE AIRCRAFT ON A CIRCLED RED "X" CONDITION AND SAID THE AIRCRAFT COULD BE FLOWN TO BEARCAT. THE OIL LEAK WAS NOT BELIEVED TO BE EXCESSIVE AND THE WHINING NOISE WAS EXPLAINED TO BE NORMAL. THE AIRCRAFT COMMANDER STARTED THE AIRCRAFT, MADE A HOVER CHECK, AND TOOK OFF FOR BEARCAT. AT APPROXIMATELY 800 FEET AND NORTHEAST OF VINH LONG THE ENGINE TACHOMETER FAILED IN THE AIRCRAFT. THE AIRCRAFT COMMANDER CHECKED HIS OTHER INSTRUMENTS AND FOUND THEM TO BE FUNCTIONING NORMALLY. APPROXIMATELY ONE MINUTE LATER THERE WAS A RAPID LOSS OF POWER CAUSING ROTOR RPM TO DETERIORATE. AT APPROXIMATELY 600 FEET THE AIRCRAFT COMMANDER LOWERED COLLECTIVE AND INITIATED AN AUTOROTATION. THE AIRCRAFT COMMANDER STATED THE ROTOR RPM INCREASED TO 294 AFTER DETERIORIATING TO 280. THE AIRCRAFT COMMANDER MADE A MAYDAY CALL TO HIS COMMAND AND CONTROL AIRCRAFT AND INSTRUCTED THE CREW TO JETTISON THE ROCKET PADS. THE MINIGUNS AND AMMUNITION COULD NOT BE JETTISONED THUS CAUSING THE AIRCRAFT TO BE HEAVY ON IMPACT. THE AIRCRAFT COMMANDER PULLED PITCH AT 10 TO 15 FEET TO CUSHION THE AIRCRAFT ON. THE RATE OF DESCENT SLOWED MOMENTARILY AND THEN THE AIRCRAFT FELL THROUGH RESULTING IN A HARD LANDING. THE AIRCRAFT COMMANDER THEN MADE A CALL TO VINH LONG TOWER AND PROCEEDED TO REMOVE RADIOS AND WEAPONS FROM THE AIRCRAFT. AN AIRCRAFT FROM THE 335TH RETRIEVED THE CREW, WEAPONS, AND RADIOS AND RETURNED THEM TO VINH LONG.\\
